Thank you for entrusting your health care to Creighton University Medical Center . We take pride in providing you with quality service in a caring environment. We'd like to do all we can to make sure your stay is as comfortable as possible and that all your questions are answered. To help you prepare, we've provided a list below of items you should and should not bring to the hospital. We've also included information on admitting hours, consent for treatment and advance directives.
What to Bring
- Your health insurance card and, if applicable, your spouse's, all insurance forms and paperwork; e.g., pre-authorizations.
- Valid driver's license, state identification card or passport.
- Advance directive documents if you have them.
- All medications you are taking in the original containers. Our pharmacy will dispense the medications you need while you are in the hospital and the original containers will provide your physician's dosing instructions.
- Any outside lab test results and pertinent X-rays not taken Creighton University Medical Center.
- Personal hygiene items.
- Clothing for your stay in the hospital (bathrobe, slippers) and for the trip home.
- Your eyeglasses, hearing aid or dentures.
- Reading and writing materials and other personal items you'll want during your stay.
- If desired, cash to pay for newspapers, gift shop articles, and other items. Not more than $20.00 is recommended.
If you are going to the Intensive Care Unit or the Cardiac Care Unit, limit personal items to those that are absolutely necessary, such as a robe, eyeglasses, dentures, hearing aid or other prosthetics. We'll provide you with slippers and grooming items. If you forget any of these items, please ask your nurse for assistance, or you may ask a family member to bring the item from home.

Creighton University Medical Center cannot take responsibility for loss of these items.
Protection of Your Personal Belongings
Please take advantage of the hospital's safe to store your valuables. Ask your nurse for assistance. The hospital cannot be responsible for the loss of money, jewelry or other valuables left in your room. Purses and wallets should be put away in a safe place or sent home with a family member.
Consent for Treatment
You will be asked to sign a consent-for-treatment form giving us permission to treat you.
